Spiritwood Stockyards (1984) Ltd. pleaded guilty to two violations of Saskatchewan's Occupational Health and Safety Regulations, 2020, stemming from two separate incidents. The first occurred on Nov. 11, 2021, when a worker was struck by a gate and seriously injured while attempting to load bulls onto a trailer. The second occurred on Jan. 26, 2022, when a worker was fatally injured while attempting to clear snow off a roof and fell through a fiberglass skylight.

Amico Contractors Inc. has been fined $80,000 after two workers were injured falling from a work platform. The workers were setting precast stairs and landings in stair shafts at a construction project in LaSalle, Ont., when they built a work platform and braced it inside the stair shaft. Shortly after the two workers climbed onto the platform to complete their work, the platform fell with both of them on it, causing injuries.

Buckhorn Sand & Gravel Inc. has been fined $200,000 after one worker was fatally injured and another was critically injured while trying to free a truck lodged on a rock.

A small construction company and its director have been fined $117,500 after pleading guilty to violations of the Occupational Health and Safety Act in an Ontario court. The fine comes after a March 2022 incident that fatally injured an employee working on a roof without guardrail protection or appropriate fall protection equipment.

Oakville Toyota has been fined $60,000 after a worker sustained injuries while performing a diagnostic assessment of a wheel bearing in a truck. The truck was on a lift in the service centre, being assessed for a defective or damaged wheel bearing when the apprentice's hair was drawn into the vehicle's rotating drive shaft.

Richvale York Block Inc. has been fined $50,000 for failing to ensure there was a guardrail at the open side of a raised platform on which a worker stood. The worker was bagging and shrink-wrapping manufactured concrete blocks when they stepped backwards and fell off the platform, sustaining a critical injury.

What happens after a fall leaves a worker hanging in their body harness? Suspension Trauma is real when a “prompt rescue” relies only on a by-chance discovery of a worker hanging in a harness and unable to self-rescue. After a fall, an unconscious worker is unable to summon assistance on their own.

The U.S. government agreed to a modest US$15 million fine for Norfolk Southern over last year's disastrous derailment in East Palestine, Ohio, and the railroad promised to pay more than US$300 million to complete the efforts to improve safety that it announced after the crash and address community health concerns.

U.S. federal regulators claim Union Pacific managers undermined the government's efforts to assess safety at the railroad in the wake of several high-profile derailments across the industry by coaching employees on how to respond and suggesting they might be disciplined. The meddling was so widespread across Union Pacific’s 23-state network that the Federal Railroad Administration reportedly had no choice but to suspend its safety assessment of the company.

Amazon’s self-driving robotaxi unit is being investigated by the U.S. government’s highway safety agency after two of its vehicles braked suddenly and were rear-ended by motorcyclists. The National Highway Traffic Safety Administration said in documents posted on its website it will evaluate the automated driving system developed by Zoox.
